Report an incident

This form is free and anonymous, and takes about two minutes. We do not ask for your name or contact details, and no identifying information is collected. You can report something that happened to you or something you witnessed.

Reports are sent securely through our forms provider and shared only with RISE Alberni, who use them to track patterns of racism and hate in Port Alberni. There is no way for us to reply, so please use Get Support if you need help.

Only two fields are required, marked required — the type of incident and what happened. Everything else is optional, and the form takes about two minutes.

Share as much or as little as you feel comfortable with. You do not need to include your name or anyone else’s.

A general area or neighbourhood is fine — you don’t need an exact address.
